How to Register online

1. Complete each field, and sign on the last page. All fields must be completed. Incomplete and/or unsigned forms will not be processed

2. For all Puppy and Basic Classes, you must attach a copy of your dog's recent vaccination record from your veterinarian which shows proof of the following:

• Puppies at 8 weeks – Distemper and Parvovirus

• Puppies at 10-12 weeks – DHPP

• Puppies at 16-18 weeks – DHPP and Rabies

          We recommend:

  • Canine Influenza and Bordetella vaccinations yearly

  • DHPP and Rabies every 1-3 years

Class Registrations must be completed online.

Your space will be saved by completing this online registration only.

** Registration Fee Must be Paid in Full Prior to the first day of Class **

              To Pay for your Class you have options.

  •  Make your check payable to IPOC:

     Mail to: IPOC Registration, PO Box 5455, Lakeland, FL 33807

• Hand deliver cash or check payable to IPOC in an envelope addressed to IPOC Registration and place in the designated lock box.

Please select the correct quantities if enrolling in more than one class

Please note that being a site member is NOT the same as being an IPOC Club Member and does not allow discounted pricing. 

You must be an IPOC Club Member to receive discounts on your class fees

IPOC MEMBERS:

• If you are using Points or Class for a Class, please fill out a Point Redemption Slip as you have in the past, this process has not changed. This slip is your payment.

• Please make sure you have available points or classes for use in the book

• In the case that you do not have anyone available to sign, put the slip in the cubby and it will be processed by a membership committe member.
